What Is Patient Information, and How Is It Protected?

When people visit health care facilities to receive medical services, they are usually required to give their health care providers relevant information that can be used to inform their treatment process. The information they give to these providers is typically referred to as patient information.

But what is patient information, and why is it important?

Patient information includes a range of different data types, such as patients’ medical history, medical test results, and insurance information. Health care professionals can use all of this data to provide better care for their patients, but patient information also has the potential to be abused by malicious parties if it is not protected. Maintaining the confidentiality of patient information is important because health records often contain extremely personal details or financial information.

Payment for all consultations, medicals, procedures, vaccinations, tests and other procedures performed at any of our centres is required on the day.

Everwell Medical Centres provides a high standard of care in well-equipped and located centres. We bulk-bills patients with valid Medicare cards. Patients without Medicare cards will incur consultation fees – you will be advised at time of check-in or consultation. Fees may be charged in some instances for certain procedures and you will be advised at time of check-in or consultation. Fees are due and payable at the time of consultation. We accept cash, credit card and EFTPOS payments. Electronic Medicare claiming is available via secure internet connection for patients who have paid their private accounts. If you are experiencing financial difficulties, please speak to your doctor at the time of your consultation.

After 11.00 pm the National Home Doctor Service can be contacted on 13 74 25. When you phone the locum service please have the following patients details ready:

• Phone number

• Full name

• Date of birth

• Gender

• Full address

• Symptoms

The National Home Doctor Service will ask for the name of the caller, any details related to finding the address, the name of the patient’s regular doctor and which Everwell Medical Centre you normally attend. A full report will be sent to your regular doctor at Everwell Mecical Centres.

IN THE EVENT OF AN EMERGENCY, PLEASE CALL 000.

Some of the doctors at our medical centres offer home visits which are generally for patients who are too ill or lack mobility to attend the medical centre. Please contact your closest Everwell Medcial Centre and our staff will advise you if your treating doctor does provide home visits or how to organise a home visit using the National Home Doctor Service (after hours).

Please enquire regarding the applicable fees. An electronic report will be sent to the practice to update your medical file.

APPOINTMENT CANCELLATION POLICY

We understand that unplanned issues can arise, and you may need to cancel an appointment. Should this occur, we respectfully ask that scheduled appointments be cancelled at least 24 hours in advanced.

Our doctors and allied health professionals want to be available for your needs, for the needs of all our patients. When a patient does not show for a scheduled appointment, other patients lose the opportunity to be seen.

It is crucial that you notify us of your cancellation, at least 24 hrs prior to your appointment time, to avoid the cancellation fee.

In virtually all cases you will be required to make an appointment with your doctor for a repeat prescription to ensure that your medication and medical condition are being managed appropriately. Your health is important and we aim to ensure good medical practice and that our doctors meet their medico-legal obligations to patients to ensure prescriptions are appropriate and that there are no risks in taking the medication.

This practice uses a “Recall and Reminder” system to provide care to patients.

This will assist in ensuring that important health checks are not forgotten and are performed on time. You will be reminded when it is time to visit your doctor for a check up or as a follow up to health risks that have previously been found. The reminder system will include check ups to help in the prevention or early detection of certain conditions such as diabetes, high blood pressure, high cholesterol and forms of cancer and immunization follow up. In some cases, the reminders can also be sent from other places, e.g. the government PAP smear register.

Even when you agree to be included in the Recall and Reminder system, you should remember when you should be tested for certain conditions and should always contact your doctor to get results of a test that has been performed.

A reminder SMS will be sent to your mobile phone advising of your appointment the night before your appointment with your doctor.

Please advise reception staff if you do not wish to happen.

Your doctor may also contact you via SMS but will not do this without prior authorisation from you.
